What happens on our first call

The first call is not a sales call. It is the intake conversation, and it is how I decide whether The Fain Standard is a fit for your situation. I have a short list of questions I ask, and I write the answers down.

I ask about the situation first. When did the discharge happen, or when is it expected. What did the discharge planner or the primary care physician say about the level of ADL support the person needs. What is the physical environment at home, and are there stairs or a bed on the first floor. What has been tried already, if anything, and what has worked or not worked. If a hospice or a palliative care service is already involved, who is the point of contact there.

Then I ask about the payer source. Private pay families and private long-term care insurance holders are the two categories FAINS serves. If the primary payer source is Medicaid or Medicare, I say so on the call and refer you to a fit-for-purpose provider before we spend more of your time. That is Pillar 7 of The Fain Standard and it saves families a week of misdirection.

I ask about the shift structure the family is imagining. How many hours a day, how many days a week, mornings or overnights or twenty-four-hour live-in. The four-hour minimum shift is Pillar 3, and if the case as described needs less than four hours per visit, I say so on the call and refer you to a visit-based provider that fits.

What I commit to on the call: a call-back or email response within one business day if you have left a message; a decision on fit before we hang up if you have reached me live; a Registered Nurse Supervisor assessment scheduled within three business days of intake if the fit is clear and the license is issued; and honest referrals if the fit is not right. I do not commit to pressure. Nothing on the first call requires you to decide anything, including whether to have a second call.

What the Registered Nurse assessment looks like

When the fit is confirmed, the RN Supervisor assigned to your case does the in-home assessment. The RN reviews the discharge paperwork, the medication list, the physical environment, the ADL support the family and the primary care physician have identified, and the family's own account of what a good day and a hard day look like now. The RN writes the plan of care. The plan of care is the working document the caregiver reads before the first shift.

The RN Supervisor is the family's clinical point of contact throughout the case. Her name and license number are on file at intake and she is the person who takes the call if something changes, if the wound gets red, if a medication is missing, if the family has questions the caregiver cannot answer. That is Pillar 1, RN supervision, and it is the pillar the other six lean on.

When to expect the caregiver introduction

The chemistry-matched pairing comes after the RN assessment. The matcher pulls two to three caregiver candidates from the license-verified roster whose skill profile matches the plan of care. The family interviews the candidates, in person or on a short video call, and picks. The first shift is scheduled after the family has picked. Nobody arrives at your door as a surprise.
