Rates published in dollars is a working commitment, not a marketing choice. Weekly transparent invoicing is Pillar 4 of The Fain Standard, and every rate below is what will appear on your invoice.
What appears on your first invoice
Your first invoice arrives on the Monday morning after the first Monday-through-Sunday week of service. The invoice has one line for each shift, showing the caregiver name, the date, the clock-in time, the clock-out time, the rate tier, and the hourly rate. If mileage was billed, it appears as its own line with the mile count. If the payment method is ACH, no fee is added. If the payment method is card, the 3 percent surcharge is a distinct line at the bottom of the invoice with the surcharge amount stated in dollars.
There are no other charges. There is no "administrative fee," no "intake fee," no "assessment fee," and no "care coordination fee." The RN Supervisor's assessment and case supervision are bundled into the hourly rate. Chemistry matching is bundled into the hourly rate. Swap fees are zero if you swap the caregiver in the first week and zero in subsequent weeks when the swap is a reasonable fit issue. If you cancel a shift with less than 24 hours notice, the four-hour minimum is billed at the tier rate. That is the entire cancellation policy. It is Pillar 6 of The Fain Standard and it is published so a family can predict it.
The published rates above are the rates on the invoice. If we ever need to change a rate for a case-specific reason (for example, a case that requires a higher acuity of nursing than the initial assessment identified), the RN Supervisor calls the family before the change appears on any invoice. The family agrees to the new rate in writing. Nothing changes on the invoice that has not been agreed to on the phone first.
Why the shift minimum is four hours
Four hours is the private duty industry standard in New Jersey. FAINS publishes it because burying it in fine print is a common tactic and one I refuse. A family calling for a two-hour morning visit deserves a straight answer, which is that a two-hour visit is a different service model than private duty, and we will refer you to a fit-for-purpose provider rather than bend our operating floor to fit a case we cannot deliver well.
Four hours is not arbitrary. It is the shift length that funds the RN supervision, the license verification pipeline, the chemistry matching, and the on-call coverage that make the other six pillars of The Fain Standard operable. A shorter shift makes those pillars financially unsupportable, and I will not offer a shift length I cannot back up with the operating structure that makes the shift valuable.
Payment methods, explained
ACH is our preferred method because it is free to the family and reliable for us. The invoice includes ACH instructions. Bank drafts settle in one to three business days.
Card payment is available with a 3 percent surcharge. The surcharge is not a markup; it is the pass-through of the card network's interchange fee. The surcharge amount is shown in dollars on the invoice so the family sees what the payment method costs.
Long-term care insurance direct pay is available for carriers whose policies permit assignment of benefits. When direct pay is available, the invoice is sent to the carrier and the family sees a copy for their records. When direct pay is not available, the invoice is formatted for the reimbursement package the family submits to the carrier. That format has been through most of the major carriers' desks and works.
The pricing pillar in context
Weekly transparent invoicing is one of seven pillars in The Fain Standard. The pricing you see on this page is the operational face of that pillar. The clinical face is RN supervision, the credentialing face is license verification, the working face is chemistry-matched caregivers, and the payer face is private pay and private insurance only. Each pillar reinforces the others. The rates on this page only make sense against the operating commitments they fund.
